What does tirzepatide cost without insurance, dose by dose?

Lilly publishes self-pay prices for Zepbound, the brand of tirzepatide approved for weight management, and they rise in only two steps. As of October 8, 2026, LillyDirect lists the following for both the single-dose vials and the KwikPen:

The lower prices for 7.5 mg and up come from Lilly’s Zepbound Journey Program, which requires refilling within 45 days of your last one. These prices cover the medicine only, and Lilly can change them.

How long do you stay at each dose?

The minimum is four weeks per step, and your plan may hold a step longer. The Zepbound label starts at 2.5 mg weekly for four weeks, then raises the dose by 2.5 mg after at least four weeks at each level. For weight management, the recommended maintenance doses are 5, 10 or 15 mg, chosen by response and tolerability.

In the SURMOUNT-1 trial, participants climbed in those 2.5 mg steps every four weeks to an assigned 5, 10 or 15 mg dose, then stayed there for the rest of the 72-week study. In practice, your provider may hold a step longer if nausea or other side effects need time to settle. Your provider sets each dose; the schedule above is context, not instructions.

A six-month estimate for the medicine

Because every dose from 7.5 mg up costs the same, your six-month total depends less on how high you go than on how fast you get there. These examples assume six four-week fills, on-time refills and the October 2026 prices.

Missing the 45-day refill window is the quickest way to raise the total. Three late fills at 10 mg would add $750 compared with the on-time price. Syringes or pen needles are extra.

FSA, HSA and insurance

Tirzepatide used to treat diagnosed obesity can usually be paid with FSA or HSA funds. IRS Publication 502 counts medicines that need a clinician’s order and weight loss treatment for a disease a physician has diagnosed. LillyDirect also marks Zepbound as HSA and FSA eligible, with eligibility varying by plan, and your plan administrator applies those rules to your receipts.

The clinic side of a tirzepatide plan is never billed to insurance at Ultimate Male, though each visit comes with an itemized receipt. If your commercial plan covers Zepbound, LillyDirect notes that the single-dose pen with a Lilly savings card may cost you less than self-pay, so check your coverage before choosing a route. Medicare and other government plans follow separate rules.

Do I need to buy syringes separately?
For the self-pay vials, yes. LillyDirect notes that a needle and syringe purchase is required, and the KwikPen needs pen needles, so add a small supply cost to your estimate.
Is tirzepatide more expensive than semaglutide?
At brand self-pay prices the two are close. Zepbound runs $299 to $449 a month depending on the dose, and Wegovy pens are listed at $349 for standard doses, so the dose you end up on matters more than the brand.
What happens to the price if I take a break?
A gap longer than 45 days between refills moves the 7.5 mg and higher doses to LillyDirect's regular prices, and restarting after a long break may also mean stepping back down to a lower dose.
