Low SHBG in men

Short answer

SHBG is the protein that binds testosterone tightly in the blood. Excess weight and insulin resistance lower it, so a man can show a low total testosterone even though his free, usable testosterone is normal. That is why free testosterone is checked before treatment is considered. Weight loss tends to raise SHBG and total testosterone together, often moving both back toward normal.

What does a low SHBG level mean?

A low SHBG level means less of your testosterone is locked onto its main carrier protein. Sex hormone-binding globulin, made in the liver, holds testosterone so tightly that tissues cannot use the bound portion. The AUA testosterone guideline breaks down where testosterone sits in a typical man: about 44% bound to SHBG, about 50% loosely bound to albumin, about 4% on another carrier and about 2% free. The free and loosely bound portions together are called bioavailable testosterone.

MedlinePlus puts it plainly: when SHBG is low, more of your total testosterone is likely free and available to your body. Labs report SHBG in nanomoles per liter against their own reference range, which varies with the test method and often with age, so compare your number with the range on your report rather than a figure from the internet.

Why does low SHBG make total testosterone look low?

Total testosterone counts everything, bound and free. When SHBG falls, there is less carrier to hold testosterone, so the bound pool shrinks and the total number drops. The free fraction, which the brain watches and which tissues actually use, can stay in the normal range the whole time.

The result is a lab report that looks worse than the biology. Two men with the same total can be in very different places:

What causes low SHBG in men?

Excess weight and insulin resistance are the most common causes. MedlinePlus lists obesity, type 2 diabetes or insulin resistance, and an underactive thyroid among the conditions linked to low SHBG, and notes that medicines containing hormones, including anabolic steroids, can affect SHBG.

The link to insulin resistance is why low SHBG often travels with a large waist, high triglycerides and a fasting glucose drifting upward. The AUA guideline adds that obese men are almost five times more likely to have low testosterone than men who are not obese, and low SHBG is part of that picture. Why does free testosterone decide the next step?

Free testosterone decides because it shows whether usable testosterone is actually low. The Endocrine Society guideline recommends measuring free testosterone when SHBG is likely to be altered, and it names obesity as one of those situations. The guideline favors measuring free testosterone by equilibrium dialysis, or calculating it from total testosterone, SHBG and albumin.

If free testosterone is normal, a low total with a low SHBG is usually not testosterone deficiency, and treating the number alone would treat the wrong thing. How does weight loss raise SHBG and testosterone?

Losing excess weight tends to lift SHBG and testosterone at the same time. A meta-analysis of 24 studies found that both diet-based weight loss and bariatric surgery increased SHBG-bound and unbound testosterone, and that the more weight men lost, the bigger the rise. Gonadotropins, the pituitary hormones that drive the testes, rose as well, which suggests the signal itself recovers.

What if you are already on testosterone therapy?

On testosterone therapy, SHBG still matters, because it affects how much of each dose ends up free. MedlinePlus notes that hormone-containing medicines can change SHBG, so a level drawn on treatment is not directly comparable to one drawn before. Is low SHBG dangerous on its own?
Not in itself, but it is often a marker of insulin resistance, so it can point to a metabolic problem worth addressing. Your provider may look at glucose, A1c, triglycerides and waist size alongside it.
Do I need to fast for an SHBG test?
MedlinePlus lists no special preparation for SHBG. It is usually drawn in the morning with testosterone, and if glucose or lipids are added to the same draw, you may be asked to fast.
Can a thyroid problem cause low SHBG?
Yes. An underactive thyroid is one of the causes MedlinePlus lists, which is one reason TSH is often checked when SHBG comes back low without an obvious weight or insulin explanation.
