What marionette lines are, and why they matter on a man
Marionette lines are the creases that run from the corners of the mouth down toward the jaw, like the hinged mouth of a puppet. On a man they often read as a fixed expression: people ask whether you are annoyed or tired when you are simply listening.
They form for three overlapping reasons. The fat pads around the mouth and chin thin out, the jawline loses some of its bone and soft-tissue support, and the tissue above slides down to sit against the crease. Men usually have fewer fine lines around the lips than women. A cadaver study of perioral skin found that women had more and deeper wrinkles there, while male skin held more oil glands, sweat glands and blood vessels. So in men, the problem around the mouth is more often a crease and a shadow from lost support than a network of etched lines.

Why jaw support changes the amount you need
This is the part many men do not expect. If the crease exists because the jaw and chin have lost support, filling the crease alone takes a lot of gel and can make the lower face look heavy, because you are adding volume to tissue that is already sagging.
The alternative is to rebuild the foundation first:

- Then the line. Once the base is set, the marionette crease often needs only a fine finishing layer of soft gel, and sometimes none.
There is no fixed syringe count, because it depends on how much support you have lost. What changes is where the product goes: men with a strong jaw may need only soft gel in the crease, while men with a softened jaw usually do better spending most of the product on the jaw and chin. Who should and should not have it
Good candidates have creases or shadows at the mouth corners, reasonable skin quality and realistic goals. Filler is not the right tool when a heavy jowl or loose neck skin is the main problem, because volume cannot lift heavy skin; in that case you will be told so and may be pointed toward a surgical opinion.
Do not have treatment if you have severe allergies or anaphylaxis, allergy to lidocaine, a bleeding disorder or an active cold sore or infection near the mouth. Tell your injector about frequent cold sores and any blood thinners or supplements that increase bruising.
Your visit, step by step, and what to expect
- Assessment. Your nurse injector studies the mouth corners at rest and in motion and checks the chin and jaw. Photos are taken.
- Plan. You agree whether this visit treats the jaw and chin, the line, or both.
- Numbing and cleansing. Topical numbing is offered; JUVÉDERM gels used here contain lidocaine.
- Injection. Small amounts of gel are placed, then molded with gentle pressure.
- Check and aftercare. You smile, talk and review the result before leaving.

The American Academy of Dermatology notes that redness, swelling and bruising tend to clear within 7 to 14 days. Avoid hard workouts, heat and alcohol for a day.
Side effects and safety
Lumps, asymmetry and prolonged swelling are the common nuisances, and they are usually correctable with massage or hyaluronidase. Blood vessels run close to the mouth corners, and the FDA warns that accidental injection into a vessel can cause tissue damage, blindness or stroke. If you notice severe pain, blanching or mottled skin, or any change in vision after treatment, call 911 or go to an emergency room straight away.
