What nasolabial fold filler treats
Nasolabial folds are the creases that run from the sides of your nose to the corners of your mouth. Everyone has them, and they deepen when you smile. With age they stay deep at rest, and on a man they can read as tiredness or sternness rather than character.
Filler for these folds places a gel or implant under the crease to lift it from below. Two kinds of product on the Ultimate Male menu carry an FDA indication for correcting moderate to severe facial wrinkles and folds such as nasolabial folds: RADIESSE, a calcium hydroxylapatite implant, and the hyaluronic acid gels in the JUVÉDERM family. Our condition page on deep smile lines in men covers why these folds form.
Why the cheek is often treated first
In many men the fold is a symptom, not the cause. As the mid-face loses fat and bone support, the cheek drops forward and down, and the skin piles up against the fixed line beside the mouth. Filling only the crease adds weight to the lower face and can make the area look heavier.
That is why your injector checks the cheek before the fold. If the cheekbone has flattened, restoring support with JUVÉDERM VOLUMA in the cheeks can soften the fold on its own, and any small amount left over is treated directly at a later visit. If your cheek is still full and the fold is simply deep, treating the fold directly makes sense.
Men whose concern is the lines running down from the mouth corners, rather than up to the nose, are looking at a different area, covered on our page about marionette line filler for men.
RADIESSE or hyaluronic acid for a man’s folds
Both work. The choice depends on how deep the fold is, how heavy the skin over it is, and how much you value reversibility.
| Factor | RADIESSE | Hyaluronic acid (JUVÉDERM family) |
|---|---|---|
| What it is | Calcium hydroxylapatite microspheres in a gel | Crosslinked hyaluronic acid gel with lidocaine |
| Feel | Firmer, gives more lift per syringe | Softer, more flexible with expression |
| Collagen effect | The microspheres act as a scaffold for your own collagen | Mainly fills; minimal collagen effect |
| Can it be dissolved? | No | Yes, with hyaluronidase |
| FDA study durability | Superior to a collagen comparator at 6 months | VOLLURE XC: improvement through 18 months in 59.4% of treated folds |
| Often chosen for | Deep, heavy folds in thicker skin | Moderate folds, first-time patients |
In the FDA pivotal study, RADIESSE was compared with collagen in opposite folds of the same patients: the mean total amount was 1.2 mL for RADIESSE against 2.4 mL for the comparator, and at 6 months 78.6% of RADIESSE-treated folds scored at least a point better. For hyaluronic acid, the VOLLURE XC directions for use report a median of 1.7 mL per fold and improvement lasting through 18 months in most treated folds.
For a man with thick, heavy skin and a deep crease, the firmness of RADIESSE often shows a change with less product, and an evidence review of calcium hydroxylapatite describes immediate volume followed by longer-term correction as the body builds collagen around it. For a man trying filler for the first time, the option to dissolve hyaluronic acid is reassuring. Our JUVÉDERM versus RADIESSE comparison goes further.
Who should not have fold filler
Filler is not right if you have a history of severe allergies or anaphylaxis, a lidocaine allergy, a bleeding disorder, or an active infection or cold sore near the mouth. A tendency to form raised scars also needs discussing before any injection. RADIESSE is not used in the lips. Men with very loose lower-face skin may be better served by a surgical opinion, and the consult will say so plainly.
Your appointment and what to expect
- Assessment. Your nurse injector looks at the fold at rest and while you smile, checks cheek support and takes photos.
- Product choice. You decide together between cheek support, direct fold treatment or both, and between RADIESSE and hyaluronic acid.
- Preparation. The skin is cleaned and topical numbing is offered. Both product families on the menu contain lidocaine.
- Injection. Product is threaded beneath the fold in small amounts, then gently molded.
- Review. You check the result sitting up and leave with written aftercare.
| When | What is normal |
|---|---|
| First 2 days | Swelling, tenderness, possible bruising beside the mouth |
| End of week 1 | Most swelling gone; the fold looks softer but not erased |
| Weeks 2 to 4 | Settled result; touch-up visit if needed |
| Later months | Gradual return; plan a review when the fold starts to deepen again |
The American Academy of Dermatology notes that redness, swelling and bruising usually clear within 7 to 14 days. Skip intense exercise, heat and alcohol for about a day. The aftercare guide has the full list.
Side effects and safety
The fold sits over the facial artery, which is why experience matters here. The FDA identifies injection into a blood vessel as the most concerning filler risk, with possible skin death, vision problems or stroke. Your injector works slowly with small amounts and watches the skin color as they go. Severe pain, white or mottled skin, or any vision change after treatment means you should call 911 or go straight to an emergency room. Other possible issues include lumps, asymmetry and, rarely, infection.
Paying for smile line filler and booking at Ultimate Male
The cost of treating nasolabial folds depends on whether the cheek is supported first, which product is used and how many syringes each side takes. Smile line filler is cosmetic, so it is generally not FSA or HSA eligible. Ultimate Male charges per visit, with no membership, and you can use CareCredit, Prosper Healthcare Lending or Afterpay through our financing page.
At our San Gabriel and Downey clinics, a nurse injector examines the fold and the cheek above it before suggesting anything, and the quote is given before treatment. If a cheek-first plan would serve you better than filling the crease, that is what you will hear. Browse the wider range of dermal fillers for men or book your smile line consultation.
