How long does BOTOX take to kick in?

Short answer

BOTOX starts working within a day or two, but you usually see it a little later. The label says the muscle begins to relax one to two days after injection and the effect builds through the first week, and dermatologists say most people see results within three to seven days. By about two weeks the result has settled, which is the right time to judge it and book any touch-up check.

What happens day by day?

The effect arrives in stages rather than all at once. The BOTOX Cosmetic label describes the muscle beginning to relax one to two days after injection, with the effect increasing over the first week. Dermatologists at the American Academy of Dermatology put the visible change for most people at 3 to 7 days.

The label’s frown-line trial data show how front-loaded the change is. At day 7, 74 percent of subjects treated with BOTOX Cosmetic were already rated responders by investigators; at day 30 the figure was 80 percent. Most of the improvement is there by the end of the first week, with a little more to come.

Why does it take days to work?

Because BOTOX does not act on the skin or the muscle directly. It works inside the nerve endings that tell the muscle to contract, blocking the release of the chemical signal the muscle needs. The toxin has to be taken up by those nerve endings first, and the effect grows as more of them are switched off.

Why wait about two weeks to judge it?

Because early impressions mislead in both directions. On day three, one side can look stronger than the other simply because it caught up first. On day five, a frown that still moves a little may finish relaxing by the end of the week.

MedlinePlus notes that it can take a few days or up to several weeks to feel the full benefit of onabotulinumtoxinA. A check at about two weeks catches the result after it has settled, while there is still plenty of the treatment cycle left to enjoy any adjustment. What should you do while it kicks in?

When is a change after BOTOX urgent?

Rarely, the effect spreads beyond the injected area. The label’s boxed warning describes symptoms such as generalized weakness, double vision, drooping eyelids, hoarseness or trouble swallowing, speaking or breathing, reported hours to weeks after injection. Trouble swallowing, speaking or breathing is an emergency: call 911. A drooping eyelid or uneven brow on its own is not an emergency, but call your injector so it can be assessed.

Why did one side kick in before the other?
Uneven onset in the first week can happen, because each muscle and injection point responds on its own schedule. Judge symmetry at the two-week check, not in the mirror on day three.
Is it normal to see nothing for the first two days?
Yes. The muscle only starts to relax one to two days after injection, and visible softening usually follows a few days later, so an unchanged face on day one or two is expected.
