Did you judge it too soon?
This is an easy false alarm to raise. The BOTOX Cosmetic label says the muscle starts to relax one to two days after injection and the effect keeps increasing through the first week. Visible results usually follow within 3 to 7 days, according to the American Academy of Dermatology.
The label’s frown-line data show the build-up clearly: 74 percent of treated subjects were rated responders at day 7 and 80 percent at day 30. Was the dose too small for your muscles?
For men, this is the first thing to suspect. In the label’s frown-line trials, which enrolled mostly women and used 20 units, the day-30 responder rate was 59 percent in men compared with 85 percent in women.
A dose-ranging trial in men points to the fix. Doses of 40, 60 and 80 units in the frown area beat 20 units on response and duration, without more side effects. Could placement or the type of line be the problem?
Yes, in two different ways. First, toxin only relaxes the muscle it reaches. A forehead line that sits higher than the injection points, or a crow’s foot that extends past the outer point, can keep moving while everything around it is still. One side may also respond differently from the other.
Second, BOTOX treats lines caused by muscle activity, which is how the label words each indication. Creases that stay visible with your face at rest are partly etched into the skin, and folds from lost volume are not muscle lines at all. Relaxing the muscle softens those, but rarely erases them. Could you be resistant to BOTOX?
It is possible but rare. Resistance happens when the body makes neutralizing antibodies that block the toxin. A manufacturer-sponsored meta-analysis of 33 trials across 10 medical and cosmetic uses found that 27 of 5,876 evaluable subjects, or 0.5 percent, developed these antibodies, and only five of them were judged to have lost their response.
The label adds that more frequent injections or higher doses may raise the chance of antibodies, and suggests the lowest effective dose at the longest feasible interval. Resistance tends to show as a treatment that used to work and gradually stops, rather than one that never worked. If it is suspected, some injectors switch to XEOMIN, which its label describes as made without accessory proteins.
Was it genuine product?
Worth asking, especially if you were treated outside a medical setting. The FDA has warned about counterfeit BOTOX from unlicensed sources, which may be ineffective or unsafe, and advises confirming that your provider is licensed and that the product comes from an authorized source.
