How long does BOTOX last for men?

Short answer

BOTOX usually lasts about three to four months, according to its label and the American Academy of Dermatology. Men sometimes get less, because larger frown and forehead muscles can outlast a dose sized for a smaller face. In a trial of men, higher doses lasted longer than the 20-unit label dose, and one small study linked high levels of exercise to a shorter effect.

What is the typical window?

About three to four months. The BOTOX Cosmetic label puts the effect on frown lines at approximately three to four months, and the American Academy of Dermatology gives the same range, adding that it sometimes lasts longer.

Pooled data sharpen the picture. A meta-analysis of four trials covering 621 people treated with the standard 20 units for frown lines found a median duration of 120 days at maximum frown among those who responded. Half lasted longer than that, and half did not, which is why your own number matters more than the average.

Why can it wear off sooner in men?

Because muscle size and dose go together. Men’s frown and forehead muscles are generally larger and stronger, and the trials behind the label were mostly women. The label reports that 82 percent of frown-line subjects were women, and at day 30 the investigator-rated response was 85 percent in women versus 59 percent in men.

A smaller response often means a shorter one, too. In a randomized trial in 80 men with frown lines, doses of 40, 60 and 80 units outperformed 20 units on peak response and on duration, with a dose-dependent increase in how long the effect lasted and no rise in side effects. Does heavy training shorten BOTOX?

Possibly, though the evidence is thin. A 2023 controlled trial split 60 people into low, moderate and high physical activity groups and gave everyone the same dose. Over 90 days, muscle activity fell significantly more in the low-activity group, and their forehead and frown lines improved more. The authors concluded that high activity seems to shorten the effect.

Keep the limits in mind. All 60 participants were women, the groups were small, and the finding has not been widely repeated. If you train hard and notice an early fade, tell your injector; that information shapes the dose and timing next time. It is not a reason to train less.

How does dose affect how long it holds?

More units in a strong muscle generally means a longer hold, up to a point. The trade-off is movement: the dose that lasts longest in a forehead can also flatten it, which is the look most men want to avoid. A careful injector balances the two, for example by giving the frown muscles a fuller dose and the forehead a lighter one. Over time, the pattern can shift. The AAD’s BOTOX FAQs note that with repeat treatment the muscle may thin, which can stretch results out. The label also says dosing more often than every three months has not been studied, so the goal is a dose that reaches the next visit rather than more frequent visits.

What else changes how long it lasts?

A few factors sit outside the dose:

  • Lines at rest versus on movement. Creases etched into the skin can show again sooner, even while the muscle is still relaxed.
  • Your own response. Individual response varies, and only a record of past treatments shows your pattern.
