What makes BOTOX look obvious on a man?
A few specific patterns, all of them avoidable. Men usually notice them on other men before they can name them:
- A flat, heavy brow. Too much toxin in the forehead muscle, the main muscle that lifts the brows, lets them sit lower and look tired or stern.
- An arched or peaked outer brow. When the center of the forehead is relaxed but the outer edges still pull, the tail of the brow lifts into a surprised, pointed shape that reads as treated.
- A shiny, motionless forehead above eyes that still crinkle, so the face no longer moves as one piece.
- A drooping eyelid, which is a placement problem rather than a dose problem.
A natural result looks like a less intense version of your own expressions. You still frown, raise your brows and squint; the lines just do not fold as deeply. The American Academy of Dermatology describes the aim as injecting just enough to relax the targeted muscles so patients keep their natural expressions.
How does placement keep your brow moving?
Placement is written into the label. The BOTOX Cosmetic label tells injectors to treat forehead lines together with frown lines to minimize the chance of brow drooping, and to keep the lower row of forehead injections at least 2 cm above the eyebrow. Frown-line injections stay at least 1 cm above the central brow.
Those rules matter because the forehead and frown muscles pull against each other. The frown muscles drag the brows down and in; the forehead muscle lifts them. Relax only the lifter and the brow drops. Relax the frown muscles too, and the balance is kept.
The label’s trial numbers show how uncommon trouble is when the rules are followed. With combined forehead and frown treatment, brow drooping was reported in 2 percent of subjects. Eyelid droop was reported in 3 percent of subjects treated for frown lines alone.
What does conservative dosing look like?
It means matching the dose to the muscle, not using the smallest dose everywhere. Men’s frown muscles are strong; a trial in men found 40 units or more in the frown area worked better than the 20-unit label dose, with no rise in side effects. Giving that area a full dose rarely makes a face look treated, because a softer scowl reads as calm, not frozen.
The forehead is different. That is where a lighter hand keeps the brows lifting and avoids the flat look. A consensus of aesthetic physicians recommended separate forehead protocols for men and women, with the dose and the injection map set by each person’s muscle tone and anatomy, so a man with a low, heavy brow is treated differently from one with a high, active forehead.

What if the result looks too strong?
It fades. The label puts the usual duration at about three to four months, and movement returns gradually rather than all at once, so an overdone result softens week by week. An outer-brow arch can often be evened out at the follow-up visit with a small additional amount.
