Ozempic Face in Men: What Causes It and What Helps

Short answer

"Ozempic face" is not a drug side effect in the usual sense. It is what rapid, large weight loss does to facial fat: the cheeks and temples deflate, skin loosens and the jawline can look gaunt rather than sharp. Men, who carry less fat in those areas to begin with, often show it sooner. Hyaluronic acid filler restores volume immediately, while Sculptra rebuilds collagen gradually over several sessions.

The short answer

“Ozempic face” describes the hollow, tired look some people develop after losing a lot of weight quickly on a GLP-1 medicine such as semaglutide or tirzepatide. The medicine does not attack the face. Fat comes off everywhere, including the small fat pads that give the cheeks and temples their shape, and the face can end up looking older and more drawn than the rest of the body.

It is common enough that plastic surgeons now write about it as its own topic. A 2025 systematic review of 23 plastic surgery publications concluded that GLP-1 medicines cause facial changes resembling advanced aging, and found that online searches for “Ozempic face” rose alongside searches for face filler. The good news is that it is treatable, and the two main approaches work in very different ways.

What actually changes in the face

Rapid weight loss affects several layers at once. A 2026 review in Dermatologic Surgery describes deflation of the superficial fat compartments, loss of deeper support, bone resorption and looser skin. It found that midface volume loss seems to happen mainly in the superficial fat, which flattens the cheek contour and sharpens the lines between facial regions.

In men, that shows up in a few predictable places:

Why lean men show it more

Two reasons. First, men tend to start with less facial fat in the places that matter. A review of facial differences between the sexes notes that the main difference between male and female midfaces may be the greater concentration of fatty tissue in the cheek and temple regions of the female face. With less padding to lose, the same weight loss uncovers bone and muscle sooner.

Second, men who were already fit, or who lose weight while training hard, often end up with very low body fat overall. A face that looked chiseled at a moderate weight can look gaunt at a much lower one.

Sculptra or hyaluronic acid filler?

The two main injectable options restore volume in opposite ways: one immediately, one gradually.

Hyaluronic acid filler is the faster fix. VOLUMA is FDA-approved for cheek augmentation and, since a 2023 approval, for moderate to severe temple hollowing in adults over 21, which makes it a natural fit for the two areas GLP-1 weight loss hits hardest. Sculptra works more slowly and more broadly. The Sculptra patient labeling describes a treatment course of up to four sessions scheduled three to four weeks apart, during which the injected particles prompt new collagen. In its earlier nasolabial fold studies, effects lasted up to 25 months in some patients. The labeling also lists small lumps under the skin as a possible side effect, a risk that rises when large volumes are placed in one spot. Many plans combine both: Sculptra to rebuild a broad foundation and a small amount of hyaluronic acid filler for a specific hollow such as the temple.

Safety and timing

Fillers are medical devices with real risks. The FDA warns that unintentional injection into a blood vessel can block blood supply, with reported complications including tissue death, vision problems including blindness, and stroke, and it advises against buying fillers online. Choosing an experienced injector who knows facial anatomy matters more than the brand in the syringe.

Timing matters too. Treating while you are still losing weight quickly can mean chasing a moving target, so many injectors prefer to wait until your weight has settled, or to start with gradual collagen stimulation and add filler later.

Get emergency care right away if, after any filler, you notice sudden vision changes, severe pain, skin that turns white or dusky near the injection site, or weakness or trouble speaking.
