The verdict: which one fits you?
NEO Slim fits men who are close to their goal weight, train at least a little, and want the area to look tighter as well as smaller. It works on muscle and fat in the same session. CoolSculpting fits men who want a pinchable bulge of fat reduced and do not care about the muscle underneath, and who accept a small risk of an unusual complication.
If you still have a lot of weight to lose, neither is the right first step. Medical weight loss comes first, and contouring comes after.
NEO Slim and CoolSculpting side by side
| NEO Slim | CoolSculpting (cryolipolysis) | |
|---|---|---|
| What it targets | Fat and muscle | Fat only |
| How it works | Targeted energy drives deep muscle contractions while heating fat cells | Controlled cooling triggers death of fat cells |
| Session length | About 30 minutes | Varies by applicator and area |
| Typical series | Four to six sessions, a few days apart | Depends on area size and pinchable fat |
| Areas | Abdomen, arms, glutes, thighs | Pinchable fat areas such as the belly, flanks and under the chin |
| Feel | Intense contractions, warmth | Cold, suction, then numbness |
| Downtime | None | Usually none; numbness and tenderness can linger |
| Notable risk | Muscle soreness for a day or two | Rare paradoxical adipose hyperplasia |
How does each one work?
NEO Slim: two effects at once
NEO Slim uses targeted energy to set off thousands of strong muscle contractions while heating and breaking down fat cells in the treated area. You lie back while the applicators work for around 30 minutes, and it feels like an intense workout without the sweat. Most men do four to six sessions spaced a few days apart, with results building over the following weeks.
Devices built on this principle have a modest but growing evidence base. A 2024 systematic review of electromagnetic muscle-stimulation devices, with and without added heat, found abdominal studies averaged a 5.5 mm reduction in fat thickness and a 2 mm gain in muscle thickness, usually after four sessions over two to four weeks. The authors also flagged gaps in how some studies reported their data, so expect a visible refinement rather than a dramatic change.
CoolSculpting: cold that targets fat
Cryolipolysis relies on fat cells being more sensitive to cold than the skin around them. An applicator draws the area in and cools it for a set time, the damaged fat cells die off over the following weeks, and the body clears them. It does not train or thicken muscle.
Side effects, including one rare risk
Both treatments are non-surgical, and most aftereffects are mild and short-lived. The one serious outlier belongs to fat freezing.
Paradoxical adipose hyperplasia
The complication most specific to fat freezing is paradoxical adipose hyperplasia, or PAH. Instead of shrinking, the treated fat slowly enlarges into a firm bulge that often matches the shape of the applicator, usually appearing months after treatment.
A 2025 meta-analysis of 28 studies and 13,078 patients estimated PAH incidence at 0.22%, about 1 in 455 patients, and noted this appears higher than manufacturer reports, though the evidence certainty was low. PAH usually needs a procedure to correct; in a 2022 case series and review, every affected patient went on to have liposuction, a tummy tuck or both to correct it. Our page on who should not get NEO Slim covers the screening questions for our treatment.
Everyday side effects and who should skip each
NEO Slim’s usual aftereffect is muscle soreness for a day or two, much like a hard training session. Devices that stimulate muscle with energy are generally avoided over metal implants and in men with implanted electronic devices such as pacemakers. Cryolipolysis commonly causes redness, bruising, numbness and tenderness that fade over weeks, and it is avoided in people with cold-triggered conditions.
When will you see a difference?
Neither treatment shows its full result on the day. With NEO Slim, firmness from the muscle work often shows first, and the slimmer contour follows over the weeks after the last session. With cryolipolysis, the dead fat cells are cleared gradually over the following weeks to months, so patience is part of the deal.
Results from both last longest when your weight stays steady. Fat cells destroyed by either method do not regrow, but the ones that remain can still enlarge if you gain weight, and toned muscle softens again without training. Our NEO Slim results timeline lays out what to expect and when.
Session counts, convenience and cost factors
Time and money both scale with the number of areas and sessions. A NEO Slim series means four to six short visits close together; read more on how many NEO Slim sessions men typically need. Cryolipolysis may take fewer visits for a single small area, but larger areas need more cycles.
We do not publish prices; the NEO Slim cost page explains what changes the total, and you get a quote before booking. Under IRS Publication 502, cosmetic procedures generally are not deductible medical expenses, so body contouring is usually not FSA or HSA eligible. Financing is available through CareCredit, Prosper Healthcare Lending and Afterpay.
How we help you choose
We start by asking where you are on the journey: how close to your goal weight, how much you train, and which area bothers you. If the honest answer is that weight loss should come first, we say so and point you to our medical weight loss program, which starts with labs and body composition analysis.
If you are a good fit, your plan names the areas, the number of sessions and what to expect week by week, at our San Gabriel or Downey clinic. Compare surgery in NEO Slim vs liposuction, or read more about NEO Slim body sculpting.
