How fast does Cialis work?

Short answer

Tadalafil (Cialis) can work within 30 minutes for some men, and the label says to take it at least 30 minutes before sex. Blood levels peak at a median of about two hours, with a range of 30 minutes to six hours. Food does not change its absorption, so you can take it with a meal. Once working, it can help for up to 36 hours.

How soon can Cialis start working?

For some men, within half an hour. The Cialis label describes a study in which men used a stopwatch to record when they first had an erection that led to successful sex. At or before 30 minutes, 52% of men on the 20 mg dose reported success, compared with 38% on 10 mg and 35% on placebo.

That placebo figure is worth noticing. A third of men on a dummy pill also succeeded, which shows that many men with ED can sometimes manage without help. The 20 mg group was 17 percentage points ahead of placebo at that point, and the label’s instruction is to take a dose at least 30 minutes before anticipated sexual activity.

Why does the full effect take longer?

Because absorption is gradual and varies between men. After a single dose, tadalafil reaches its highest blood level anywhere from 30 minutes to six hours later, with a median of two hours. A man at the slow end of that range may find the first attempt underwhelming if he tries too soon.

Time after a dose What is happening
30 minutes Some men already respond; the label’s minimum lead time
About 2 hours Median time to peak blood level
Up to 6 hours Latest time to peak in the label’s data
Up to 36 hours Erectile function still better than placebo

If the first try or two are disappointing, give it more lead time rather than doubling the dose. Our answer on why ED pills sometimes do not work covers the other usual reasons.

Does food change how fast Cialis works?

Barely, if at all. The label states that the rate and extent of tadalafil absorption are not influenced by food, so it may be taken with or without a meal. MedlinePlus says the same.

That sets it apart from sildenafil. The Viagra label says a high-fat meal delays sildenafil’s peak by about 60 minutes and lowers that peak by 29%. For men who would rather not plan sex around an empty stomach, that difference matters. Our comparison of sildenafil and tadalafil sets out the other trade-offs.

Alcohol is a different story. The tadalafil label warns that substantial drinking, about five units or more, combined with the pill can lower blood pressure and cause dizziness. Our answer on alcohol with ED pills explains the limits.

How should you plan around the timing?

Use the long tail rather than racing the clock. Because one dose can help for up to 36 hours, you can take it earlier in the day, or even the evening before, and stop watching the clock. A few practical points:

  • Take it at least 30 minutes ahead, and allow two hours or more for the first few tries.
  • Take no more than one dose a day, as the label states.
  • Arousal is still needed; the pill does not start an erection by itself.
  • If timing still feels like a chore, ask about daily low-dose tadalafil, which keeps a steady level so timing stops mattering.

Our guide on how to take sildenafil and tadalafil covers the routine in more detail.

How long will one dose keep working?

Up to 36 hours. The label reports better erectile function than placebo at 24 and 36 hours after a dose. It does not mean a 36-hour erection, only a window in which arousal can lead to one. Our answer on how long Cialis lasts explains what that window means for nitrates and side effects.

How Ultimate Male helps you time it

At our San Gabriel and Downey clinics, a provider reviews your health, medicines and goals before recommending tadalafil, sildenafil or another option, and sets the dose and schedule with you. If on-demand timing does not suit your life, daily dosing is discussed as an alternative.

Does a higher dose work faster?
The label's only onset data by dose is the 30-minute study, where 52% of men on 20 mg and 38% on 10 mg reported success. Your provider sets the dose based on how well it works and how you tolerate it.
Do I need to be aroused for Cialis to work?
Yes. Tadalafil has no effect without sexual stimulation, so it will not cause an erection on its own once it kicks in.
Can I take it on an empty stomach for a faster effect?
There is no need. The label states that food does not influence the rate or extent of absorption, so an empty stomach does not speed it up.
