What changes when a man tries injectables for the first time
Three things set a man’s first aesthetic plan apart: stronger facial muscles, a brow shape that should not be altered, and the value of starting small. Most published dosing and most before-and-after expectations come from studies and practices built around women. Applied unchanged to a man, they can leave lines barely softened in one area and a brow lifted into an arch in another.
A good first plan corrects for both. It treats one area with a dose matched to your muscle strength, keeps your natural brow line, and books a review before anything else is added. That review is what turns a first treatment into information you can use for every visit after it.
How male anatomy changes the dose and the pattern
Male anatomy changes two things at once: how many units a muscle needs and where they go.
Stronger muscles usually need more units
Men generally have larger, stronger facial muscles than women, so the same label dose relaxes them less. The clearest evidence comes from the frown lines between the brows. In a randomized dose-ranging study of 80 men, doses of 40, 60 and 80 units in that area worked better and lasted longer than the 20-unit label dose, without more side effects. The authors concluded that men benefit from starting at 40 units or more there.
That does not mean every man needs a large dose everywhere. Forehead muscles, crow’s feet and frown muscles differ in strength from man to man, and a heavy brow or hooded lids can call for less, not more. Keep the brow flat, not arched
A typical male brow sits low and runs fairly straight across the bony ridge above the eye. Many female brows sit higher with an arch toward the outer third. Injection patterns designed to lift and shape a female brow can give a man a raised outer tail, which looks surprised or overdone on a male face.
For a man, the goal is to soften lines while the brow stays exactly where it is. In practice that means:
- sparing the lowest band of the forehead muscle so it can keep holding the brow up;
- spacing small doses evenly across a broad forehead rather than concentrating them at the outer edges;
- balancing forehead and frown-line treatment so neither muscle group drags the brow.

Why the first visit treats one area
Starting with one conservative area lets you learn how your face responds before committing to more. For most first-timers, that area is the frown lines between the brows or the forehead, whichever bothers you more. Crow’s feet can follow once you know your response.
A single area has three advantages for a first visit:
- A cleaner read. If one side lifts more or the effect wears off early, you know exactly which dose to adjust.
- Lower stakes. A slightly undertreated area is easy to top up. An overtreated one has to wear off over months.
- A baseline for the future. Your injector records the units and points used, so the next visit starts from your own data.
The American Academy of Dermatology notes that most people see results within 3 to 7 days, with improvements lasting about 3 to 4 months and sometimes longer. That gives you a full cycle to judge before deciding on a regular schedule.
The two-week review, then filler
The plan includes a return visit at about two weeks, when the result has settled and any unevenness is visible. At that review the injector checks symmetry, brow height and how much movement you kept, and makes small touch-ups if needed. Only then does filler come into the conversation.

Waiting matters because relaxed muscles change how folds and shadows look. A man who planned filler for lines around the mouth may find he wants less, or a different area, once the upper face is softer. Filler is also a different decision: it adds volume rather than relaxing muscle, and the AAD fillers FAQ says redness, swelling and occasional bruising tend to clear within 7 to 14 days. Safety, side effects and who should wait
Wrinkle relaxers are well studied, but they are medicines with real risks. Bruising, a mild headache and a temporarily heavy brow or eyelid are the most common problems. Men with nerve-muscle conditions such as myasthenia gravis, an infection at the injection site or an allergy to botulinum toxin should not be treated without specialist input, and some antibiotics and muscle relaxants can strengthen the effect, so bring your medicine list.
Filler carries a different risk. The FDA calls accidental injection into a blood vessel the most concerning complication: rare, but it can cause tissue damage, vision problems including blindness, or stroke. That is one more reason to treat filler as a medical procedure done by trained staff, and to start it only after you know how your face responds.
Trouble swallowing, speaking or breathing after a wrinkle relaxer, or sudden vision changes after filler, are emergencies. Call 911 or go to the ER.

- How many units do you expect my muscles need, and why?
- Where will you inject to keep my brow at its current height?
- Which single area should we start with?
- When is my review, and is a touch-up included in the plan?
