Why is your own production quiet when you stop?
While you are on TRT, your brain senses plenty of testosterone and turns down LH and FSH, the two pituitary signals that drive the testes. Testosterone production and sperm production both slow as a result, which is why the AUA guideline says testosterone should not be used by men who are trying to conceive.
When you stop, nothing switches back on at once. The testosterone you were taking has to clear first, then the pituitary has to restart LH and FSH, and then the testes have to respond. Blood work tracks each of those steps. Our page on what happens when you stop TRT covers the symptoms side.
A practical recovery lab schedule
No guideline sets an exact calendar for recovery labs, but checks at roughly 4, 8 and 12 weeks after your last dose has cleared give a clear picture without over-testing.
| When | What to check | What you hope to see |
|---|---|---|
| At or just before stopping | Total testosterone, LH, FSH, hematocrit | A baseline, with LH and FSH usually suppressed |
| About 4 weeks | LH, FSH, morning total testosterone | LH and FSH starting to rise from the floor |
| About 8 weeks | LH, FSH, morning total testosterone; estradiol if symptoms suggest it | LH and FSH in or near the normal range, testosterone climbing |
| About 12 weeks | LH, FSH, morning total testosterone, hematocrit; semen analysis if fertility is the goal | Testosterone holding in range without treatment |
When the clock starts depends on your form of testosterone:
- Injections. The Depo-Testosterone label gives injected testosterone cypionate a half-life of about eight days, so levels fall over several weeks after the final shot.
- Pellets. The Endocrine Society guideline notes pellets hold testosterone in range for three to six months, so recovery starts only once they are spent.
- Kyzatrex. The capsule is taken twice a day, so its effect fades within days of the last dose and the recovery clock starts soon after.
Which numbers show natural production is restarting?
The order of events tells the story. The first sign is LH and FSH climbing back into the lab’s reference range; the MedlinePlus encyclopedia lists 1.8 to 8.6 IU/L for LH in adult men, though every lab sets its own range. Testosterone follows, rising on its own in early-morning, fasting draws.
| Pattern at 8 to 12 weeks | What it suggests |
|---|---|
| LH and FSH normal, testosterone normal | Recovery under way or complete |
| LH and FSH normal or high, testosterone still low | The testes are slow to respond, or a testicular problem existed before TRT |
| LH and FSH still low, testosterone low | The pituitary signal has not restarted yet, or an underlying cause is still present |
Recovery returns you to where you started, not higher. If your testosterone was low before TRT because of something that still exists, such as excess weight or a pituitary problem, it will settle low again once you are off. Our pages on the LH blood test and the FSH test for men explain each marker.
How long can recovery take?
Timelines vary widely from man to man. The American Society for Reproductive Medicine says sperm should return to the semen within about 3 months of stopping testosterone in many cases, with no permanent damage. The AUA is more cautious for men with prior testosterone or anabolic steroid use, noting that return of sperm can be highly variable and take up to two years.
Hormone recovery shows the same spread, which is why repeat labs tell you more than any fixed date. A slow first month is not a verdict on the next three.
What if production does not come back?
If LH and testosterone are still low after about three months, it is time for a focused evaluation rather than more waiting. Your provider looks at whether a cause of low testosterone was present before you started, checks prolactin and other pituitary markers where needed, and reviews what you want from here.
The options depend on that goal. For men who want fertility, the AUA notes that HCG, alone or with a selective estrogen receptor modulator, has been shown to help restore testosterone production and sperm in men who previously used testosterone or anabolic steroids; see HCG therapy for men. If you are planning a pregnancy, our page for men trying to conceive covers the wider picture. For men whose symptoms return and fertility is not the priority, restarting treatment is a reasonable choice, and our page for men restarting TRT after a break covers that path.
How Ultimate Male supports coming off TRT
If you decide to stop, your provider builds the plan with you: when the last dose clears, when each recovery draw falls, and which markers to add for your goals. Draws are done on site in San Gabriel and Downey, with results usually back in 24 to 48 hours, so you see each step as it happens.
Stopping is a legitimate choice, and you will not be pushed back onto treatment. If recovery stalls, you get a clear explanation of why and what the options are, including testosterone therapy if that is what fits. If you can, book the free 10-minute call before your last dose, so your baseline is drawn at the right time.
