What under-eye filler does
Under-eye filler softens the hollow, sometimes called the tear trough, that runs from the inner corner of the eye down and out toward the cheek. When that groove deepens, it casts a shadow that makes you look tired even after a full night of sleep. A small amount of soft gel placed deep, close to the bone, lifts the floor of the groove so less shadow forms.
The amounts are small. In the FDA study of the gel approved for this area, the median total was 1.0 mL per side across the first session and an optional touch-up. Our page on hollow under-eyes in men explains the causes, from genetics and age to low body fat.
Hollows versus bags: who is a candidate
This is the decision that matters most, and it is why the consultation takes longer than the injection.

The FDA study was built the same way. It excluded people with large lower-lid fat pads, darkness caused by pigment, thin atrophic skin in the area and significant mid-face volume loss, as listed in the VOLBELLA XC summary of safety and effectiveness. If your cheek has flattened, lifting it with JUVÉDERM VOLUMA in the cheeks may do more for the under-eye area than filling the hollow directly.
Filler is also off the table if you have severe allergies or a history of anaphylaxis, allergy to lidocaine or the bacterial proteins used to make hyaluronic acid, or a bleeding disorder.
Which product is used under the eyes
Of the products Ultimate Male carries, only the soft hyaluronic acid gels in the JUVÉDERM family are suited to the under-eye area. Within that family, JUVÉDERM VOLBELLA XC holds an FDA approval for the under-eye hollow in adults over 21, granted in May 2021, with results lasting up to 12 months. Your injector will confirm the exact gel at your consultation.
RADIESSE and Sculptra are not used here. An evidence review of calcium hydroxylapatite advises against placing it around the eyes, where nodules are more likely, and neither product can be dissolved. The appointment, step by step
- Lighting check. Your nurse injector looks at the area under overhead and side light, with you looking up and straight ahead, and takes photos.
- Plan. You agree on whether to treat the hollow, the cheek or both, and how much to place at the first visit. Conservative is the rule here.
- Preparation. The skin is cleaned well and topical numbing is offered. The gel itself contains lidocaine.
- Placement. Gel is placed deep, near the bone, with a fine needle or blunt cannula, a little at a time.
- Review and aftercare. You sit up, check the result and leave with cold-compress and sleep instructions. A review is booked at about four weeks.
The bruising and swelling timeline
Under-eye skin is thin and bruises easily, so plan treatment at least two weeks before any important event. In the FDA study, 40.2% of participants reported bruising after the first treatment and 41.7% reported swelling. Most cleared quickly, but some lasted longer:

Side effects and safety
Beyond bruising and swelling, the main local issues are lumps, a faint bluish tint if gel sits too close to the surface, and puffiness that lingers. Because hyaluronic acid can be dissolved with hyaluronidase, most of these can be corrected.
The area around the eyes carries the most serious filler risk. The FDA states that injection into a blood vessel can cause vision problems, including blindness, and stroke. Any change in vision, eye pain or skin turning white or gray during or after treatment is an emergency: call 911 or go to the nearest emergency room immediately.
