What BOTOX does for forehead lines
Horizontal forehead lines come from the frontalis, the broad muscle that lifts your eyebrows every time you look surprised, concentrate or talk with your face. Over years, those creases deepen until some stay visible at rest. BOTOX Cosmetic (onabotulinumtoxinA) temporarily relaxes the muscle so the skin above it creases less.
The BOTOX Cosmetic label covers moderate to severe forehead lines in adults. The American Academy of Dermatology says most people see results within 3 to 7 days, lasting about 3 to 4 months and sometimes longer.
The heavy-brow problem in men
The frontalis is the only muscle that lifts the brow. Weaken it too much, or in the wrong place, and the brow sinks. Men are especially vulnerable: a typical male brow already sits lower and flatter than a female brow, often with more weight in the upper eyelid. A dropped male brow reads as tired or stern, the opposite of what most men want.
Skilled forehead treatment in men therefore looks different:
- Injections stay higher. The lowest band of the frontalis, just above the brow, is usually spared so it can keep holding the brow up.
- Lateral movement is preserved. Keeping some lift at the outer brow avoids a flat, heavy look.
- Smaller doses spread wider. Several small points across a broad male forehead give a softer result than a few large ones.

Why forehead units are paired with frown-line units
The label recommends treating forehead lines together with glabellar (frown) lines, specifically to minimize the risk of brow ptosis. The recommended dose is 20 units in the forehead plus 20 units between the brows, 40 units in total.
The logic is mechanical. The frown muscles between your brows pull the brows down and in. If the forehead is relaxed but the frown muscles are left at full strength, they win the tug of war and drag the brow lower. Relaxing both keeps the balance. That is why most men who come in for forehead lines end up treating frown lines at the same visit.
Why men usually need more units
Men tend to have larger, stronger facial muscles than women, and most cosmetic trials enrolled mostly women. In the label’s glabellar line trials, for example, 82% of subjects were women.
The clearest male-specific data come from the frown area. In a randomized dose-ranging study of 80 men, 40, 60 and 80 units in the glabella were consistently more effective and longer lasting than the 20-unit label dose, without more side effects, and the authors concluded men benefit from starting doses of at least 40 units there. There is no equivalent male study for the forehead, so forehead dosing in men is adjusted to muscle strength, forehead height and, above all, brow position. Who it suits, who should skip it, and how the decision is made
Forehead treatment suits men with lines that bother them and a brow that sits comfortably above the eye. Men who already have hooded lids, or who unconsciously raise their brows to see better, may get a heavier look and need a very light touch or a different approach.
It is not for anyone with an infection at the site or an allergy to botulinum toxin, and men with nerve-muscle conditions such as myasthenia gravis need specialist input first. Some antibiotics in the aminoglycoside group and muscle relaxants can strengthen the toxin’s effect, so bring your medicine list.

Forehead BOTOX is a cosmetic treatment. Under IRS Publication 502, cosmetic procedures generally are not medical expenses, so FSA and HSA funds usually cannot be used.
