BOTOX for frown lines in men

Short answer

The 11 lines between the brows come from the corrugator and procerus muscles, and they can make a man look angry or tired even when he is not. BOTOX relaxes those muscles. The label dose is 20 units, but a study in men found 40 to 80 units worked better and lasted longer. Results start in days and typically last 3 to 4 months.

What the 11 lines are, and why they matter for men

The two vertical creases between your eyebrows, often called the 11 lines, come from the corrugator muscles that pull the brows together and the procerus muscle that pulls them down. Every squint, scowl and moment of concentration folds the skin in the same place. Over time the folds stay put.

For men, these lines carry social weight. Colleagues and partners read them as anger, stress or exhaustion, even when you feel fine. Softening them is often less about looking younger and more about not looking annoyed in every meeting.

BOTOX Cosmetic (onabotulinumtoxinA) was first approved for exactly this area, and frown lines remain the most studied use. It temporarily blocks the nerve signal to those muscles, so they contract less and the skin above them smooths.

Why men usually need a higher glabellar dose

The BOTOX Cosmetic label sets the glabellar dose at 20 units, given as 4 units at each of 5 points. That dose was established in trials where 82% of participants were women.

Male frown muscles are generally bigger and stronger, and the dose needed reflects that. In a randomized, double-blind study of 80 men, doses of 20, 40, 60 and 80 units were compared. The 40, 60 and 80 unit doses were consistently more effective than 20 units in peak response, improvement from baseline and duration, and side effects did not increase with the higher doses. The authors concluded that men benefit from starting doses of at least 40 units.

How long a first treatment lasts

The label explains that the effect usually begins one to two days after injection and increases in intensity over the first week. It puts the duration for glabellar lines at about 3 to 4 months. In the label’s trials, 80% of treated people were responders at day 30, falling to 25% by day 120, which shows the gradual fade.

For a first treatment, two things affect how long you get. Dose matters: in the male study, duration rose with dose, so a cautious first dose may wear off sooner. Muscle strength matters too, since stronger frowners tend to see movement return earlier. Who it suits and who should not have it

Skip it if you have an infection at the site or an allergy to any botulinum toxin product. Men with myasthenia gravis or other nerve-muscle disorders need specialist advice first. Very deep, etched lines at rest may soften but not vanish, and sometimes need additional treatment later.

The visit, side effects and safety

Your appointment includes a look at your frown in motion and at rest, a short discussion of how much movement to keep, then 5 or more small injections between and just above the brows. It takes minutes. Stay upright for a few hours afterward and avoid rubbing the area.

Most men go straight back to work. Small bumps at the injection points flatten within an hour or so, and any bruise can usually be covered or simply left alone. Plan the visit at least two weeks before an event so the full effect is in place.

The most common side effect in the label’s glabellar trials was eyelid ptosis, a drooping upper lid, in 3% of people, compared with none on placebo. Bruising, headache and mild swelling at the points can also occur. A drooping lid usually improves as the effect wears off.
